Please know your call is important to us. We receive a large number of calls daily but respond to them within 24 hours. If you call and do not get a person, please leave a message. We return all messages within 24 hours on each business day. When you call our office you will be directed to our scheduling desk where you will be asked several questions regarding your mental health needs and your insurance. You will need to have your schedule and insurance card available when you call the office.

If your child has Medicaid, please note that we can not make an appointment until we have received the Medical referral from your child’s primary physician.

 

You will be asked questions such as:

  • Insurance Type
  • Contract/Member ID
  • Client Name
  • Client Date of Birth
  • Insurance Holder Name
  • Insurance Holder Date of Birth
  • Primary Care Physician
  • If your child has Medicaid, please note that we can not make an appointment until we have received the Medicaid referral from your child’s primary physician.

Checking In to Your Appointment

To make things easier for all of our clients, you can now access the check-in (sign-in) form directly from the website. Your ongoing scheduler will email you this information, too! Please save it on your phone and share it with any other guardians who might be bringing a child client. When you arrive at our office, wait in the parking lot. Pull up the website below and sign in. This will help our counselor identify you. Your counselor will come out to the parking lot to pick you up. If you do not do this, they will not know you arrived, and you risk missing your appointment. NOTE: If you are at CULLMAN, complete this form from the lobby of POB1 (Professional Office Building 1). The counselor will come to the ground lobby and ask for you. You will type “lobby” in the question about the car type.
Steps:
2. Complete the form using the CLIENT’s information.
3. Type the access code provided by the office via text (it is also on the welcome screen of your client portal) into the box when prompted.
4. Select your counselor.
5. Press “Sign In”.

    As a part of GCC’s strict adherence to the ethical code of the American Counseling Association, our staff does keep limited pro bono slots available. We have partnered with a local nonprofit organization to offer additional financial assistance for those who meet the requirements through our Mental Health For All program. In order to be considered, please complete the form. This form must be completed and processed before assistance can be applied to your account. The level of payment assistance will be determined after consultation with our billing department.

    No-Show Cancellation Appeals Form

    Appointments must be cancelled at least 48 hours in advanced of your appointment or you will be billed for your appointment.

    If you must cancel your appointment less than 48 hours beforehand, due to illness or serious emergency, you may complete the appeals form below to be considered for a reduction (in the form of a partial refund to your account). Please plan ahead and cancel at least 48 hours in advance to avoid the fee. Unless you make other arrangements for payment (with cash or check), we will automatically bill the fee to your credit card on file the day and time of your appointment. If we have made an error in scheduling or you believe you deserve special consideration for a “No-Show / Late Cancelation Fee”, please complete the form below. Your request will be reviewed and you will receive a decision via email.
